PART ONE: WEBSITE PRIVACY POLICY

What Information We Collect: When you visit bloomwell-care.com, basic information may be collected automatically through Squarespace, including your IP address, browser type, pages visited, and time spent on the site. This information is used solely to understand how people use the site and to improve your experience.

If you fill out the contact form on this site, we collect your name, email address, phone number, and the content of your message. This information is used only to respond to your inquiry.

Third Party Services: This site is hosted by Squarespace. Their privacy policy is available at squarespace.com/privacy. Any booking links on this site direct you to a third-party HIPAA-compliant client portal with its own privacy practices.

Contact Form Confidentiality: Please be aware that the contact form on this website is not HIPAA-secure. Do not include sensitive personal health information in any message submitted through this form. Secure communication with your therapist takes place exclusively through the HIPAA-compliant client portal.

Children's Privacy: This website is intended for adults only. We do not knowingly collect information from anyone under the age of 18.

Changes to This Policy: This privacy policy may be updated from time to time. Any changes will be reflected on this page with an updated date.

PART TWO: NOTICE OF PRIVACY PRACTICES (HIPAA)

This notice describes how your health information may be used and disclosed and how you can get access to this information. Please review it carefully.

Who We Are

Bloomwell Care is a covered entity under HIPAA. Your therapist is a Licensed Professional Counselor (LPC), License No. C7178, providing outpatient mental health therapy services via telehealth in the State of Oregon.

Your Protected Health Information

Protected Health Information (PHI) includes any information that identifies you and relates to your health, treatment, or payment for services. This includes session notes, diagnosis, treatment plans, billing records, and any other information you share in the course of your care.

How We Use and Disclose Your Information

Your PHI will be used and disclosed only in the following circumstances:

Treatment — Your information may be used to provide, coordinate, and manage your care. With your written consent, your therapist may communicate with other providers involved in your treatment.

Payment — Your information may be used to process payments for services rendered, including generating superbills for insurance reimbursement purposes.

Healthcare Operations — Your information may be used for internal practice management purposes such as quality improvement and training.

Required by Law — Your information may be disclosed without your consent in the following circumstances:

  • There is reasonable belief that you are in imminent danger of harming yourself or others

  • There is reasonable suspicion of abuse or neglect of a child, elder, or dependent adult

  • A court order or legal process requires disclosure

  • As otherwise required by state or federal law

Your Rights

You have the right to:

  • Request a copy of your health records

  • Request corrections to your health records

  • Request restrictions on how your information is used or disclosed

  • Receive a list of disclosures made without your authorization

  • Receive this Notice of Privacy Practices in paper form upon request

  • File a complaint if you believe your privacy rights have been violated

Filing a Complaint

If you believe your privacy rights have been violated, you may file a complaint with Bloomwell Care directly or with the U.S. Department of Health and Human Services Office for Civil Rights at hhs.gov/ocr. You will not be retaliated against for filing a complaint.

Effective Date

This Notice of Privacy Practices is effective as of January 1, 2026 and will remain in effect until replaced.
